Correct

   Open pneumothorax is quite uncommon, but when they do occur a sucking chest wound may result in significant ventilation / perfusion mismatch. An effective "shutter valve" or 3 sided dressing is difficult to achieve in an extensive injury of this type. Clearing the pneumothorax by the 3 sided dressing technique requires strong respiratory effort whereas the combination of positive pressure ventilation and suction ICC ensures clearence.
